ETVA is open 8AM-5PM, Monday through Friday. Patients are seen by appointment only. ETVA welcomes referrals for vascular or general surgical issues and colorectal surgery difficulties. Please call 903-595-2636 (option 2) to schedule an appointment. An evaluation will be scheduled with one of our three board-certified surgeons. Patients will need to complete paperwork prior to being seen by the physician. Paperwork can be completed by accessing the Online Forms.
At ETVA, we accept most major insurance plans. As a service to you, ETVA will file a claim to your insurance company. Our business office staff will verify your insurance benefits prior to being seen. Patients are expected to pay any co-pay or deductible remaining at the time of service. If surgery is recommended, a pre-payment may be required. If you have any concerns of your financial responsibility, please call our Business Office Supervisor at 903-595-2636, ext. 105.
ETVA accepts 
For prescription refills, please contact your pharmacy during regular business hours. The pharmacy will contact our office for authorization of your refill.
If the Tyler Independent School District is closed, ETVA will also be closed.
